Introduction
These Terms and Conditions ("Terms") govern the relationship between Dr. Sanjoy Sengupta ("Surgeon") and the patient ("Patient") receiving cardiovascular and cardiothoracic surgical services. By seeking treatment, the Patient acknowledges and agrees to these Terms.
1. Medical Treatment and Advice
- The Surgeon will provide medical treatment and advice based on the Patient's condition and current medical standards.
- The Patient has the right to refuse or discontinue treatment at any time.
- The Surgeon reserves the right to refuse treatment if, in their professional judgment, the Patient's condition is not suitable for treatment or if the Patient is uncooperative.

2. Consent for Treatment
- The Patient or their authorized representative must provide informed consent for any surgical procedure or treatment.
- The Patient acknowledges that they have been informed about the risks, benefits, and alternatives of the proposed treatment.

3. Medical Records and Confidentiality
- The Surgeon will maintain the confidentiality of the Patient's medical records in accordance with applicable laws.
- The Patient has the right to access their medical records upon request.

4. Payment and Insurance
- The Patient is responsible for payment of all services rendered, including any co-payments, deductibles, or non-covered services.
- The Patient must provide insurance information and cooperate with the Surgeon's office to facilitate claims processing.
5. Cancellation and No-Show Policy
- The Patient is responsible for keeping scheduled appointments. Failure to attend may result in a no-show fee.
- The Patient must provide 24 hours' notice for cancellations or rescheduling.

6. Liability and Indemnification
- The Surgeon is not liable for any complications or outcomes arising from the Patient's failure to follow medical advice or instructions.
- The Patient indemnifies the Surgeon, their staff, and affiliates against any claims or losses arising from the Patient's non-compliance with these Terms.

7. Governing Law and Jurisdiction
- These Terms are governed by and construed in accordance with the laws of India.
- Any disputes will be resolved through litigation.
8. Changes to Terms
- The Surgeon reserves the right to modify these Terms at any time.
- Changes will be effective immediately and will be communicated to the Patient.

By seeking treatment, the Patient acknowledges that they have read, understood, and agree to these Terms.
